After reviewing your intial information and answers to some of the questions, I would like to prescribe you a GLP-1 (ex: semaglutide OR tirzepatide) medication because I think it has the lowest side effect/risk profile while providing the best chance for weight loss. If you are not ok with either of these, please let me know. Due to concerns over availability/access to the medications, I would like to prescribe you a compounded version of the semaglutide OR tirzepatide. I would like to remind you that, while compounded medications are common, they are not subject to FDA approval and are considered "off-label". *It is legal and common for physicians to prescribe a drug for off-label use when it is determined to be medically appropriate. In fact, according to the Agency for Healthcare Research and Quality, one in five prescriptions today are for off-label use. While you have the right to request the medication be sent to any pharmacy, there is no guarantee that the pharmacy you choose will be able to compound this medication (most pharmacies like CVS, Walgreens, RiteAid, Amazon, DO NOT offer compounding), and you may be required to pay any difference in cost. If no objections, your prescriptions will be sent to the compounding pharmacy, and shipped directly to you.Please read the following about all GLP-1s COMMON NAMES (in alphabetical order): Liraglutide (Saxenda) Semaglutide (Ozempic, Rybelsus, Wegovy) Tirzepatide (Mounjaro, Zepbound) HOW THEY WORK: Increasing insulin production from the pancreas. Decreasing glucagon release after a meal. Glucagon triggers your liver to store fat. Slows gastric emptying, which will make you feel ‘full’. BENEFITS: GLP-1's have been shown to help with weight reduction when combined with lifestyle medications such as exercise and reduction of caloric intake. RISKS: **May decrease effectiveness of oral contraception. To prevent pregnancy consider using a barrier device such as condom. Medicines in the GLP family have caused thyroid tumors in lab mice. It is not yet known if they will cause thyroid tumors or medullary thyroid carcinoma (MTC) in people. No studies have confirmed a linkage between GLP-1's and thyroid tumors in humans.   • In case you were not aware, compounded tirzepatide will no longer be available very shortly. While I recognize that you would like tirzepatide, I would prefer to go ahead and make a modification to another GLP-1 medication, semaglutide. I do think semaglutide is a very good medication, and has the same side effect profile as tirzepatide, therefore you should not notice any significant difference. The dosing between the two are NOT equal (meaning 1mg of semaglutide does not equal 1mg of tirzepatide), therefore please understand that I have extensive experience prescribing both of these medications, and will aim to choose a dose which appropraite for you, regarless if this is your first time taking a GLP-1 medication, or if you are already being prescribed this medication (not having to 'start all over'). Let's start/switch you over to semaglutide. Ok?

Then, upload a current photo and provide your current weight. Scenario 2: If the photo you uploaded is recent, but you’ve lost a significant amount of weight in the past 12 months, please document your current weight and your maximum weight in the past 12 months. Additionally, upload a photo from when you were at your maximum weight during that period. Scenario 3: None of the above apply to you.   • Hello, the photo is a little older but not too old. I was about 185 lbs in the photo and lost some weight when I was on Zepbound. I'm currently around 150 and have gained a little since my insurance changed and I am no longer able to get the Zepbound covered.

  • I am aware the trizeptide is no longer available and I am okay with semaglutide. I was on 10mg trizeptide last month and I hope I can start at a higher dose of the semaglutide so I don't have to start over if that's possible? I have gained some weight since my insurance changed and could no longer get Zepbound covered.

  • Dr. Shellenberger who started working with me said I could start at a higher dose since I was most recently on the 10mg dose of the tirzepatide. I was a BMI of 30 but my insurance changed recently which is what brought me to Amble. Which is why I would like to start semaglutide at an equivalent dose to the tirzepatide 10mg.

Here are the official safety guidelines and if you ever need them keep them handy: Restarting Therapy: • If ≤2 doses are missed, restart the same dose weekly. • If 3–4 doses are missed, restart dose at 50% of previous dose weekly. • If ≥5 doses are missed, restart therapy at starting dose once weekly. for safety reasons and to avoid unnecessary side effects, this is the schedule that is recommended for restart/skipped times thank you Given your BMI of 23 it is also not safe to be on high doses.. side effects can be quite terrible. 0.5 mg would probably be the highest I would recommend. Please let us know your choice.

As it has been more than 4 weeks since your last prescription, please upload an image of your most recent prescription for weight-loss medication. This must include your name, the name of the medication, the dose of the medication and the date prescribed. The GLP-1 medications (semaglutide) and the GLP-1/GIP medications (tirzepatide) are not the type of medications that can be paused or stopped for a period of time without requiring a dose adjustment. Your body loses tolerance very quickly once the medication has been stopped and restarting at the same dose very frequently results in severe side effects. If you've missed two or fewer doses, we can continue at your previous dose. If you've missed between two and four doses, we will need to cut your dose in half and restart from there. If you've missed more than 4 doses, we will need to restart from the next to lowest dose (0.5mg for semaglutide and 5mg for tirzepatide).
